I am contacting you today due to an unauthorized transaction that was made with my prepaid visa debit card ending numbers 9751 on 7/8/2018. I am now attempting to stop and be reimbursed for this transaction which i did not authorize. I was told to contact this department with the following information in order to do so. I do apologize for any inconvenience this may have caused and will take steps in the future to avoid something like this from happening again. Thank You!
