I have been in an accident and my claim towards my compensation fund has been closed, without receiving any feedback as to why.
I am still unfit and my doctor and resubmitted my documents BUT TO DATE I STILL HAVE NOT RECEIVED ANY FEEDBACK. whenever I do call, the phone just rings 012-3261570

I am writing this in regard to my claim 408263.

It has been 9 months since I have reapplied and I have still not received, any feedback regarding this matter.
I have a loan to my name and have to pay monthly instalments.

My doctors & hospital bills are still outstanding. Dr S Baba have re-submitted my claim forms again.
Since my accident things have been difficult financially and have returned back to work.
My health conditions, have been getting worst. From my doctors, report attached you can see.

Therefore I request you to please release my compensation immediately as I am dire need of it or alter someone to contact me regarding this matter and let me know what is going on.
I have contacted the department several times, trying to get some feedback, but still have not received any information.

For your reference I have enclosed the documents in this regard. Please resolve this issue and ensure the payment.

I look forward to a quick resolution to my claim. Hope you will look into the matter and release the payment soon.
