If you have any choice at all - avoid Dr. Stephen Kashian.

I was referred, took time off, psyched myself up for this unpleasant procedure, took special medication that I had to take in order to get it, and ended up being insulted, strung along, and eventually worse from his bad medicine..

He likes money more than taking true care of his patients.
