I have same complaints as many others. Instead of receiving my check I get another 4581 form, and a few words saying I made a mistake on my form that I have to call to resolve matter. And like others I get the runaround, one recording to five, another recording to five more. Pure BS. I triple checked my form for mistakes and found none. A GRADE SCHOOLER CAN DO THESE SIMPLE FORMS. WHAT I THINK IS THEY LOST MY ORIGINAL 4581 FORM. NOW IF I HAVE TO REDO THE FORM ICAN'T RECALL THE NAMES OF ALL THE RECEPTIONISTS I SPOKE TO, I CAN BARELY RECALL THE BUSINESSES I WENT TO.I really have no idea how to proceed, I cannot for the life of me get through on that lousy phone system they have. it's a joke.
