Short term disability paperwork

I filed with sedgwick on june 30th 2017. still waiting on paperwork for the doctor to fill out to complete my claim for short term disabilitly following surgery. my doctor appointment is today and I still don't have the paperwork they were supposed to send me within 2 business days. today is july the 6th.
