Monthly premium scam

My name is Shannon Harrell. Upon canceling my debit card I had a threatening email from HII Insurance Solutions that my Insurance would be canceled. My Insurance is with Anthem Blue Cross! I then researched my monthly statements to learn that HII Insurance has been charging me $69.00 per month for a premium I do not even have! How did they get my Debit card information. They have scammed me out of over $1,500 dollars!

Please let me know what I can do to recover my money and put these scammers out of business!
