I'm Sylvia Thusi a mother of 3 children of late Hitler Dludlu with ID Number 660907 5423 089.

May I have your assistance in this case of late Hitler Dludlu he passed away on the 14th June 2020. We sent all documents to your office together with his HR forms.

When I called your office December 2020. They told me that they were busy processing, I phoned in February 2021 they told me that they didn't recieved other documents. I sent it personally. In July last year she told me that my affidavit wasn't clear In August I sent it. In October she asked bank details of my first born Mpilenhle Dludlu and details of the disease family with the same surname I've done that. I phoned for a feedback she told me that that his brother know nothing about his funeral service. I explained that he was having a funeral policy with his Union NUMSA as I mentioned on the application form. Nhlanhla Dludlu is his brother in other house, he was only person left on his house.

I gave her his uncle contact details as a person who was signing him in Mortuary and transporting him from here to KZN.

When I phoned in September she asked for B1 doctors letter together with my other Affidavit I sent it. In October she asked for details of Sbongile she's having a child with Hitler. Fortunately enough we search her son on Facebook we find his details and I sent it to your office. In November when I phoned she told me that she going to send me Waiver form for Sandile's l sent that form to them there are staying at KZN Durban

They sent them by post to your office when I phoned on the 15th December she told me that she's didn't recieved it.

My main problem is that I am not working since 2018 my last born is going to high school this year 2022 tomorrow they want registration fee for admission I don't have money.
Your office phoned Sandile when I phoned her today she's told me that she's waiting for Sandile's waiver forms.
