unauthorized use of debit card

On May 23, 2014, There was an unauthorized charge on my debit card from Netspend at 3:15 am for the amount of $14.99.
First of all, how did anyone get access to my account? Second, I never authorized anyone to pull any money out of my account.
Now, I need to know how do I get my money back? And how do I press charges?
