Waiting since last October 2015 for my rebate

I submitted my Verizon wireless Costco rebate form last October 2105 to get my Costco $200.00 gift card. Today is February 5th,2016 and not only have I not received the rebate gift card online they do not have a record of my submittal. This is disgusting and Costco needs to make this right by putting pressure on their vendors today! I have been a loyal Costco member for over 26 years because of their customer service. Tell me why I should not shift over to Amazon?
