I was injured on the job, my right foot was crushed between 2 pieces of machinery and resulted in numerous surgeries and the amputation of all 5 toes on my right foot. My injury occurred in April 2015 and I returned to work in November of 2015. I did everything exactly as I was advised by my case worker, I even contacted her the day I was released so that no payments would overlap. Since this time I have been told numerous stories from different case workers, assuming we could get someone on the phone. Originally I was told I would be paid for the amputation of my toes, loss time(which I received weekly, there were several times we had to call to get checks) future medical and etc. I received a check in the mail for 5.440 which I had no clue what it was for therefore I did not cash the check and still have it. I was able to speak to Carol Lovett and she first told me that "We do not operate that way and do not just mail out checks" Then after she further reviewed my case she said she did find whee the check was sent but she was not certain as to exactly what it was for. In November of 2016 I started trying to contact Karen Eckard, who was my case manager and finally in February I find out that Karen and carol Lovett (her supervisor whom we had also spoken to on several occasions) were no longer employed there. At this time we spoke to Richard Weaver who told us that we would be assigned a new case manager, Matt Cohen, who we have yet to hear from. Mr Weaver is also extremely hard to contact and has told us numerous different stories related to the pay out of our claim. He is going through all the same stories as the previous 2 workers and when we tell him we know that is not true and are able to disprove what he is stating he avoids the questions all together and now we can not contact him at all and have yet to hear from the person he said would be handling our claim. We have been trying to settle this claim since 11/2015 and have had no results.
