I purchased an iPhone 6 at the kiosk in the Costco Wholesale store in Green Oaks Township in MI on 2-24-15. I signed a 2year activation through Verizon Wireless at the kiosk with the salesperson. I mailed in the required documentation to Stuart Lee Rebates in Lynnwood WA on 3-3-15 requesting the $20.00 rebate for purchasing the phone at Costco Warehouse and signing the 2 year contract with Verizon (all required for the rebate). Today is 6-17-15 and I still have not received my rebate. I went on the Stuart Lee Rebate website and filled out the form to contact them regarding this but the site refused to process it as they "didn't recognize the vendor". I tried to locate a number to call them but have been unsuccessful. This is extremely frustrating and unfair to consumers.
