Denied permanent card

I have had a Green dot card for many years, and I was receiving a retirement direct deposit into the account every month. When I could not access funds in May, 2015, I realized that I had not received a new card and the old card expired 4/15. I was told I needed to purchase another card to get my direct deposit put in the account. I called the number for activation of the temp card and was informed that I would be receiving a permanent cared in 7-10 days, it never arrived and I was directed to go online and apply for a permanent card. After filling out forms with the same information, I was denied a permanent card because the system said it could not verify my information. Nothing has changed with my personal info, and I am very upset with this entire system.
